Flo-Test, Inc. — SMS Messaging Terms

ABOUT THIS PROGRAM
Flo-Test, Inc. ("Flo-Test," "we," "us") operates an operational SMS messaging program tied to active oil-and-gas field-service work. We send texts to two groups of recipients:
- Flo-Test employees — job assignments, Job Safety Analysis (JSA) signature requests, Driver Vehicle Inspection Report (DVIR) red-tag / out-of-service alerts under 49 CFR Part 396, and PIN / password setup links.
- Customer site representatives (points of contact, foremen, approvers) on active work orders — crew-arrival notifications, service-completion follow-ups, ticket-approval links, and JSA acknowledgment requests.
All messages are operational. We do not send marketing or promotional text messages under this program.
HOW YOU OPT IN — AND CONFIRM
Your mobile number is provided to Flo-Test directly — employees during onboarding in the Flo-Test Operations App, and customer representatives when scheduling service. At that time you are shown or read this disclosure:
"By providing your phone number you agree to receive operational SMS from Flo-Test. Message frequency varies. Standard message and data rates may apply. Reply STOP to opt out, HELP for help."
We then send you a one-time confirmation text, and you are not enrolled until you reply YES:
"Flo-Test: Reply YES to receive operational texts about your job/work — crew arrival, completion, approval links, safety alerts. Msg freq varies. Msg&data rates may apply. Reply STOP to opt out, HELP for help."
Only after you reply YES will Flo-Test send operational messages. If you do not reply YES, you will not receive further texts.
MESSAGE FREQUENCY
Message frequency varies with active job activity — typically 1 to 10 messages per week per recipient.
MESSAGE AND DATA RATES
Message and data rates may apply. Standard carrier rates apply based on your mobile plan.
OPTING OUT AND GETTING HELP
Opt out at any time by replying STOP (or STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT) to any message. You'll get one confirmation that you've been unsubscribed, and we will send no further messages unless you re-enroll by replying START.

PRIVACY
